- Wallerian degeneration after stroke: a new prognostic factor?Publication . Soares-Fernandes, J; Beleza, P; Ribeiro, M; Maré, R; Almeida, F; Rocha, JWallerian degeneration (WD) after ischemic stroke has been associated to persistent motor impairment, but signal intensity changes on conventional magnetic resonance imaging (MRI) are generally not detected until four weeks after the event. We report a 54 year old male patient, referred to our hospital for sudden-onset left hemiparesis. Cerebral CT showed right fronto-parietal infarct (middle cerebral artery stroke). We performed two CT control, which revealed no haemorrhagic transformation. MRI, obtained 13 days after the onset, demonstrated the infarct, mainly subcortical, extending throughout fronto-temporo-parietal areas and restricted diffusion in the ipsilateral corticospinal tract. In conclusion, WD is apparent on diffusion-weighted imaging within two weeks of stroke, allowing a better prognostic evaluation of recovery. The abnormal signal should not be misinterpreted as new ischaemic lesions.

Sometimes, the findings are the result of the epileptic activity, and not the cause reflecting the pathophysiologic changes during epileptic activity. In this paper we describe a patient with status epilepticus who has developed hemicerebellar involvement contralateral to a frontal epileptogenic focus. This phenomenon of Reverse Crossed Cerebellar Diaschisis is rare and has been describe only in Single Photon Emission Computed Tomography (SPECT).

The most recent minimally invasive approach is natural orifice transluminal endoscopic surgery (NOTES), which limits morbidity because this surgery does not require incision. This article reviews the history, development, and current and future applications of NOTES in the field of urology.
- Operação de Hartmann: casuística do Departamento de CirurgiaPublication . Martins, S; Carneiro, T; Toscano, A; Soares, V; Kock, P; Mesquita-Rodrigues, Aoperação de Hartmann tem como vantagem a segurança do procedimento cirúrgico, e como principal desvantagem a necessidade de uma 2ª. intervenção cirúrgica para conclusão do tratamento. Os autores realizaram um estudo retrospectivo dos doentes submetidos a operação de Hartmann num período de 7 anos (1997-2003). (Introdução) A operação de Hartmann foi inicialmente descrita em 1921 para o tratamento de doentes com carcinoma do recto. Hoje em dia é principalmente utilizada sempre que está contra-indicada uma reconstrução num só tempo. Consiste na ressecção do segmento envolvido pela patologia, com encerramento do topo distal e construção de uma colostomia proximal terminal. Apresenta como principais vantagens a segurança do procedimento, isto é o facto de permitir remover a fonte do problema ao mesmo tempo que elimina o risco de uma anastomose sob condições desfavoráveis. A principal desvantagem resulta de necessitar de um 2º. tempo cirúrgico 'major', geralmente 6 ou mais semanas depois da 1ª. intervenção, que poderá ser dificultado pelo facto de o coto distal ser difícil de identificar e estar retraído, e ainda de ser preciso realizar a desmontagem da colostomia. No entanto alguns doentes ficam privados da 2ª. cirurgia, e a operação de Hartmann dá origem a um estoma definitivo. (Conclusões) As principais indicações para a operação de Hartmann, no Departamento de Cirurgia do Hospital de S. Marcos, foram o adenocarcinoma do cólon e a diverticulite complicada. Esta cirurgia foi realizada principalmente em doentes idosos. No entanto é relativamente segura, tendo-se verificado uma baixa morbilidade, e a mortalidade apresentada resultou da história natural da doença e não da intervenção cirúrgica. Só cerca de 50% dos doentes foram submetidos a reconstituição do trânsito, sendo este um procedimento cirúrgico seguro.

- Isolated velopalatine paralysis associated with parvovirus B19 infectionPublication . Soares-Fernandes, JP; Maré, RA case of isolated velopalatine paralysis in an 8-year-old boy is presented. The symptoms were sudden-onset of nasal speech, regurgitation of liquids into the nose and dysphagia. Brain MRI and cerebrospinal fluid examination were normal. Infectious serologies disclosed an antibody arrangement towards parvovirus B19 that was typical of recent infection. In the absence of other positive data, the possibility of a correlation between the tenth nerve palsy and parvovirus infection is discussed.

- A twenty-year survey of dermatophytoses in Braga, Portugal.Publication . During the last few years, Braga has suffered a radical change from a rural to a cosmopolitan life-style. METHODS: A statistical study of dermatophytoses and the etiology of their causative agents was performed by a retrospective survey carried out among patients of Hospital de São Marcos, Braga, Portugal, from 1983-2002. In this study, a total of 10,003 patients were analyzed. RESULTS: Over this period the frequency of dermatophytoses, as defined by the recovery of a dermatophyte in culture, was found to be 23.6%, whereas nondermatophytic infections accounted for 7.0%.
