- Avaliação das Anomalias do Metabolismo da Glicose Após a Gravidez Complicada por Diabetes GestacionalPublication . Matos, C; Pereira, Mntroduction: Gestational Diabetes Mellitus (GDM) is associated with an increased risk of maternal Diabetes Mellitus (DM). Our objective was to analyse post-partum reclassification anomalies, and its relation with maternal history, pregnancy data and neonatal outcomes. Methods: Retrospective analysis of women with gestational diabetes between the year 2000 to 2007, aged from 16 to 45 years old; 47.2% had positive familiar history of DM and 9.1% had past history of GDM. Other maternal and gestational characteristics were collected; newborn weight and morbidity and reclassification results were evaluated. Statistical analysis was based on descriptive methods and Spearman correlation, Kruskal-Wallis, Mann-Whitney, Qui-square and Fisher tests, and multivariate analysis by logistic regression. Results: 424 women; 69.6% underwent reclassification; 6.1% were diagnosed impaired fasting glucose, 12.5% impaired glucose tolerance and 3.7% DM. A relation was established, between progression to impaired glucose metabolism and family history of diabetes, elevated BMI before pregnancy, earlier gestational age at diagnosis and need for insulin therapy. Discussion: Only elevated BMI before pregnancy seems to predict progression to altered glucose metabolism. Therefore, it is important to develop preventive strategies for obesity and be aware that every woman with GDM undergoes reclassification tests.
- Petrified auricular cartilages pointing the diagnosis of post-partum hypopituitarism in an encephalopathic patientPublication . Machado, A; Lopes, M; Ferreira, CTrue ossification of ear auricles is exceptional. We present the first case linking this finding to post-partum hypopituitarism. A 57-year-old female presented with a 2-day history of fever, headache and behavioural disturbances. Brain magnetic resonance imaging was normal. Since cerebral spinal fluid could not be obtained, she was treated empirically for a meningitis. A urinary tract infection was subsequently identified as the cause of fever but when she improved she remained apathetic. At this time petrified auricles were noticed; histological examination revealed true ossification. Endocrinological screening showed partial hypopituitarism and thyroid autoimmune disorder. Initial symptoms could be dated to the birth of her last child 15 years before, with breast feeding difficulties, loss of body hair, and transient amenorrhoea. The absence of overt peripartum bleeding, resumption of menses 1 year later, preservation or recovery of some hypophyseal function, and presence of an associated autoimmune thyroid disorder and of hypophyseal tissue in a normal sella turca, all suggest lymphocytic hypophysitis rather than Sheehan syndrome as the primary disorder. Of the 15 patients reported to date with auricular ossification, two had Addison disease. The present case suggests that low cortisol is the key factor in this clinical finding.

Matos, C; Pereira, ML; Guimarães, JTSince prolactin (PRL) (a hormone produced by the anterior pituitary) was first identificated, the existence of hyperprolactinemic syndrome has been recognized. Main symptoms are galactorrhea, oligomenorrhea, amenorrhea and infertility in women and decreased libido and impotence in men. Macroprolactinemia reflects the predominance of circulating forms with reduced bioactivity not associated with typical clinical manifestations of hyperprolactinemia. It is identified by immunoassays commonly used in clinical practice, resulting in hyperprolactinemia. Polyetilenoglycol (PEG) is the most used method that removes PRL from serum. It is likely that the phenomenon of macroprolactinemia is consistently underestimated and unrecognized. Manufacturers of immunoassays for PRL have been slow to incorporate in the literature, validated protocols, and data related to the interference of PEG. From a clinical and biochemical point of view and, the main concern should be to avoid unnecessary investigation and treatment.

- Incisão sublabial versus columelar no acesso trans-septal para cirurgia trans-esfenoidal microscópica da hipófise. Estudo retrospectivoPublication . Guimarães, J; Santos, J; Miranda, D; Carvalho Moreira, F; Marçal, N; Pereira, G; Marques, O; Almeida, R; Vilarinho, S; Frias, A; Alegria, C; Pratas, RObjectivos: Comparar a incisão sublabial com a incisão columelar no acesso trans-septal para cirurgia trans- -esfenoidal microscópica da hipófise, quanto às complicações perioperatórias e sequelas observadas. Desenho do estudo: Estudo retrospectivo. Material e métodos: Revisão dos registos clínicos de 50 doentes submetidos a cirurgia trans-esfenoidal microscópica para ressecção de adenoma da hipófise, no Hospital de Braga, entre Julho de 2007 e Novembro de 2010. Foram colhidos e avaliados dados demográficos, co-morbilidades pré-operatórias, classificações histológica e imagiológica do adenoma, duração da cirurgia, duração do internamento e complicações per(pré)- e pós-operatórias. O seguimento dos doentes foi entre um e seis meses. Na análise estatística foi utilizado o programa SPSS® Statistics 17.0. Resultados: 50 doentes, 32 (64%) do sexo feminino e 18 (36%) do sexo masculino, com idades compreendidas entre os 16 e 87 anos, média de 50 anos. 40 (80%) casos tratavam-se de macroadenomas e 10 (20%) de microadenomas. 15/47 (31.9%) doentes foram submetidos a cirurgia trans-esfenoidal transseptal através de incisão columelar e 32/47 (68.1%) doentes através de incisão sublabial. Verificaram-se complicações oronasais num total de 5/14 (35.7%) doentes no grupo em que foi utilizada incisão columelar e 14/29 (48.3%) doentes no grupo em que foi utilizada incisão sublabial (p>0.05). Verificou-se que doentes com excesso de peso tiveram mais frequentemente complicações (p<0.01). Não se verificaram diferenças estatisticamente significativas na taxa de doentes com complicações de acordo com a idade, género, incisão cirúrgica utilizada, tipo de adenoma, ou em doentes com outras co-morbilidades pré-operatórias. A duração da cirurgia e do internamento não tiveram diferenças estatisticamente significativas relativamente à incisão cirúrgica utilizada. Conclusões: As incisões sublabial e columelar na cirurgia trans-esfenoidal mostraram, neste estudo, não afectar as taxas de complicações per(pré)- e pós-operatórias. The differential diagnosis of inflammatory arthritis that appears in a child with autoimmune thyroid disease managed with antithyroid drugs is discussed.
